National Healthcare Decisions Day

National Healthcare Decisions Day (NHDD) exists to inspire, educate, and empower the public and providers about the importance of advance care planning. NHDD is an initiative to encourage patients to express their wishes regarding healthcare and for providers to respect those wishes, whatever they may be.

The Conversation Project

The Conversation Project is a public engagement initiative of the Institute for Healthcare Improvement with a goal that is both simple and transformative:

To have every person’s wishes for end-of-life care expressed and respected.

Substance Abuse Rehab for Veterans

While other segments of the population might turn to substance abuse for a variety of reasons, veterans often turn to substances like alcohol, tobacco, and prescription drugs when suffering from difficulties like post-traumatic stress disorder (PTSD) and homelessness.

The good news is that if you are a veteran who struggles with substance abuse, you can receive a variety of services for free from the VA including help detoxing, treatment in facilities, individual or group therapy, medications, and other services.

Plan Your Lifespan

No one knows what their future health holds, but every­one wants to have their voice heard through­out their lifespan.

This website will help you plan for health events such as hos­pital­izations, falls, and memory loss that may happen as people get older. This plan­ning differs from end-of-life care and wills.

The PREPARE programs were developed by Dr. Rebecca Sudore, MD out of a deep passion to help empower people and their family, friends, and caregivers to obtain and understand medical information and to make informed medical decisions.

These programs are free to the public. PREPARE is a program that can help you make medical decisions for yourself and others, talk with your doctors, and get the medical care that is right for you
